There was one aspect of this new age which was characteristic of all fields of study however, and that was the idea that the human being is a creature of immense faculties, great ability and endless potential in both constructive and destructive endeavors. As a result of this humanistic movement of the 16th and 17th centuries, many thinkers tried to understand humans more thoroughly by determining what basic characteristics are inherent in all men and women. More specifically, they tried to determine how and why humans evolved to form civilized societies, and what motivated them to do so. In addition, as a continuation of the work done by pre-enlightenment thinkers such as Aristotle and St. Thomas Aquinas, scholars debated as to whether or not humans were naturally endowed with the faculty of reason, morality, and whether humans had any natural rights. Determining how humans exist in a state of nature became an important factor in determining why man evolved to form civilized society. John Locke and Thomas Hobbes were two great thinkers of this time who formed radically contrasting theories about the state of nature and mankinds emergence from it. In this paper, it is the intent of the student to show how evidence-based practice in primary care nursing correlates with the steps in the nursing practice for Neonatal Care (NICU) using the total patient care model. Care Delivery Model The team care delivery model seems to be the most relevant model for working in the Neonatal ICU to insure patient safety. When infants are in the intensive care unit they have different needs than infants who are in a regular nursery. In order to make sure that infants, parents, and family needs are met while the infant is in the ICU, a team provides around the clock care for each infant. According to our textbook, the RN is in charge of planning and providing the tasks to the other team members as to what is needed for each infant. The RN creates a coll aborative environment with all of the team members and is responsible for scheduling each member of the team. As a primary care nurse, being a part of the team is essential because the primary care nurse is the first one to see the infants in the ICU on a daily basis. Working as part of a team makes sure that the infants are cared for during breaks, lunches, and other situations that may come up for individual nurses. How Steps in Nursing Process Relate to Evidence-Based Process (A) Assessment The assessment area for evidence-base nursing in the literature is largely based on assessing the overall NICU programs. In some of the literature, it is shown that nurses are not always knowledgeable about evidence-based practice which may inhibit their ability to use certain interventions (Smith, Donze, Cole, Johnston, & Giebe , 2009). Using evidence-based practices has been shown to increase the level of care for patients by about 30% (Smith, Donze, Cole, Johnston, & Giebe , 2009). Allen an d Chubb (2010) add that using evidence-based assessment can help to redirect the needs of a Neonatal unit to improve care on every level. Russ, Dougherty, and Jagadish (2010) state that evidence-base practice can help nurses narrow the knowledge gap when they are working in the NICU which enhances their ability to o their jobs. (D) (Nursing) Diagnosis In diagnosis, evidence-based nursing provides an understanding of the interventions that have been used to assist in many conditions. D’Agostino (2010) suggests that the literature provides many cases of information about premature babies and how to diagnose their problems. The process of diagnosis was discovered through a series of evidence-based articles in electronic journals. The research provided a concrete foundation for working with premature babies and it provided the information that the nurses needed to proceed with a specific process. FISH! -Stephen Lundin, Harry Paul, Jon Christensen ‘There is always a choice about the way you do your work, even if there is not a choice about the work itself’ A very powerful statement. As a person with a work experience of a couple of years this sentence would have changed the way i approached my work and things related to me. That’s why this book connected with me from the very beginning.The story of the book revolves around the story of Mary Jane Ramirez and her workplace and how an accidental visit to a ‘Fish Market’ changed the way an entire department at an office as well she changed their style of working at work and improved relationships at home. As in the book the main character Mary – a person good at heart & mother of 2 children – after the death of her husband lives a life wherein she is bothered more about the job security rather than being firm and taking risks. With her transfer as a head of Operations department, a dr eaded department in every sense for being unresponsive, unpleasant, negative.In fact it is referred to as toxin energy dump. She feels the need to bring about a change in the department for its revival. A visit to a Fish Market and in particular a store named popularly known as the Pike Place Fish Market where she sees that the workers selling Fish enjoy the work they do & have a lot of fun doing so and also play along the way and involve people around in their activities. Herein the character of Lonnie comes into picture. He is worker at the store and enjoys his job. At this point Lonnie explains Mary the reason for the ‘energy’ visible at the place. He says that there are 4 ingredients that make this place run.He tells her only 1 principle followed and that of ‘Choose Your Attitude’. Mary impressed with the advice sought more data from her boss Bill (a person she did not have friendly relationship) and got a few insights from David Whyte’s poem whi ch changed her for the better. She knew it was time for change. She went through even more materials like that by John Gardener and Sarah Ban Breathnach’s Simple Abundance. At the end of it she knew that had to be the leader who believed in herself and bring about the change. The very concept of ‘Choose Your Attitude’ was applied by her to her own thinking and then explained to her staff.The staff agreed about the problems of the department and gave reasons for their behavior. But she had shown them a way of revival. The staff too felt the need for the change and found a leader who actually cared for them. They decided to take it in a positive way and the result was seen in the elevator where a poster of Choose Your Attitude was put up. The change had begun. With added confidence Mary along with her kids Brad and Stacy visited the market again when they learned the second ingredient of ‘Play’. A value which was displayed by a kid, which adults need t o learn enjoy their work.This taught that a lot can be learned from kids. Having fun doesn’t take the seriousness out of the business but one enjoys what one does and achieves targets. The third ingredient in creating a high-energy world famous market was ‘Make Their Day’. It was showcased by a co-worker at the store named Wolf giving the trio a fish each and was also seen when people were involved in their daily activities of the shop. The people enjoyed being a part of it to the fullest. That indeed made their day and made them happier. Also focusing ones attention on ways to make another person’s day provided a constant flow of positive feelings.The fourth ingredient ‘Be Present’ by displayed by the workers in the way they interacted with the customers and remained attentive and interested in answering the queries of their clients. Mary decided to take her staff for the field trip and made them have a firsthand experience of the fish market and her entire staff was inspired and they were given time to think about their learning and think about the ways to implement it at workplace. The fact that a few of the staff members actually visited the fish market showed that they were influenced by the idea. They took their families along with them and they enjoyed the time spent.With most of the employees inspired by process it was decided to create teams based on each of the 4 ingredients and they were given all needed the required support by Mary. Each of the teams presented their reports on each of the topics. Each team drafted their topic by enlisting what all would be the benefits of Play and how these could be implemented in the Office. The team Play made the members take part in a game in which circles cut from coloured paper and people stepped on them along with the music. The benefits and implementation was shown through this.The make their day team divided the entire group into various teams. Each team was given fif teen minutes to develop strategies for supporting and enhancing the work of a key group of people. A customer survey was used as input data for the same, which took many of them by shock. The winning team which gave the best suggestion was given symbolic mementos. Next The Present Moment Team which made the employees to relax and had inspired reading took place in the entire hall. Also a number of experiences of individual employees were shared. The team decided to resolve on a few changes for better functioning.Finally The Choose Your Attitude Team made a presentation which was brief and to the point. They decided to distribute books which would inspire individuals and have discussions on them. The strategy got implemented throughout the next year. Mary on her part too decided against quitting her job as she was confident of bringing about a change to the department and in its people. Exactly a year later the situation had changed and the department was one of the most sought after places in the company. Mary was awarded with the Chairwoman’s Award for her work. The icing on the cake was the finding of a life partner in Lonnie.The character played of Mary is a person who has lost a few qualities along the way due to family circumstances. She rises from it and makes the workplace better for her as well as other employees present. She experiences change first hand with the relationship between her boss and also her employees. Lonnie’s character is also central to the passage. He explained Mary the various aspects of the 4 ingredients so that she could implement it at her workplace. The book has a message in each and every incidence. The frustration with jobs is experienced by all. The principles which have been mentioned have truly enriched me.The principles of ‘Choose Your Attitude’,’ Play’, ‘Make Their Day’, ‘Be Present’ are something which i will like to apply in my daily activities in office as well as college. The approach of not quitting a job in which one is unhappy but making it a better place for oneself and the people around in utmost essential. The issues mentioned are experienced by each one of us including me and the 4 ingredients provide the key to success. Along with that the resolution of Mary Jane to bring up her kids is commendable. It teaches not to give up irrespective of any calamity and keep striving to achieve ones goals.Even in my daily life I get sadden by events but the book has taught me to cheer myself up and try to change certain bad tendencies in me to avoid repeat of events. Chronic Obstructive Pulmonary Disease Essay

Introduction â€Å"Chronic obstructive pulmonary disease (COPD) is a lung ailment that is characterized by a persistent blockage of airflow from the lungs† ( World Health Organisation (WHO) website, nd). This report will examine the causes, symptoms and treatment of COPD. It will discuss current statistics on COPD and some of the current treatments available. Multiple sources were used including websites, leaflets and government documents. Over 440,000 people in Ireland are thought to have COPD ( Living with COPD website, nd). By 2020, it is predicted to be the third leading cause of death worldwide (Living with COPD website, 2013). A recent COPD audit in Europe discovered that 50% of sufferers will die or be readmitted to hospital within a 3 month period (Living with COPD website, 2013). Approximately 1400 people die every year from the condition (Oireachtas website, 2013). Ireland has one of the highest incidents of death in Europe from COPD (Living with COPD website, 2013). COPD is an irreversible lung disease which causes permanent lung damage. In COPD, the airflow to the lungs become narrower and blocked and breathing becomes difficult (COPD Support website, nd). COPD is an umbrella term for Chronic Bronchitis and Emphysema (Living with COPD website, nd). Causes The single biggest cause of COPD is smoking. 80-90% of people with the condition have COPD due to smoking. (COPD Support website, 2009). Other causes are air pollution, exposure to fumes or dust particles at work, passive smoking, recurring respiratory infections in early life and a rare genetic disorder called Alpha-1 Antitrypsin Deficiency (National Heart, Lung and Blood Institute website, nd). Diagnosis and Symptoms A breathing test called Spirometry is used to diagnose COPD (COPD FOUNDATION Website, nd). The test measures how well a person breathes. In Ireland, access to Spirometry Tests is not available nationwide and tests must be carried out in hospital (Oireachtas website, 2013). Early diagnosis can greatly slow down the condition (National Health Service website, 2012). Often COPD is diagnosed after a person has an  exacerbation of the condition (American Thoracic website, nd). An exacerbation of COPD means the  symptoms have become more severe. It is generally caused by an infection in the lungs and hospitalisation is required which leads to diagnosis of the condition ( American Thoracic website, nd). Symptoms of COPD include wheezing, persistent cough with mucus that will not go away, breathlessness completing everyday tasks and difficulty taking deep breaths (American Lung Association website, nd). Symptoms typically start to emerge from the age of 40 years onwards (Living with Copd website, 2013). Long term complications associated with COPD include, weight loss and swallowing issues known as Dysphagia (Queensland Health and The Australian Lung Foundation, 2012). Swallowing issues can arise as the patient can often breathe when eating, which causes food to enter the lungs. This can lead to reflux, indigestion problems, difficulty in finishing meals and lung infections (Queensland Health and The Australian Lung Foundation, 2012). Stages There are 4 stages of COPD: Mild, Moderate , Severe and Very Severe (End Stage) (COPD International website, nd). Mild: The person may have slight breathlessness and may or may not have a cough but is unaware they have COPD (COPD International website, nd). Moderate: Breathlessness carrying out day to day tasks, may or may not have frequent cough with mucus. At this stage the condition is normally diagnosed  as symptoms become more severe (COPD International website, nd). Severe: COPD greatly affects the quality of life, physical activity is limited, exacerbations of COPD are much more frequent (COPD International website, nd). Very Severe/End Stage; At this stage oxygen is required continuously, any exertion at all is difficult. Chronic Respiratory Failure and/or Heart Failure and death occurs (Kenny,T 2011). This knowledge would enable a Healthcare Support Worker to identify the early signs of the condition, have an increased awareness of a patient with a persistent cough, frequent colds or chest infections and encourage the patient to seek medical help for a diagnosis. Treatments Stopping smoking is critical in treating COPD, as giving up smoking greatly prevents the condition worsening. Inhalers are generally used to control and relieve symptoms of COPD. Short-Acting Inhalers are used to alleviate breathlessness, Long-Acting Inhalers which work over 12 hours to relieve symptoms, and Steroid Inhalers also assist breathing (British Lung Foundation website, nd). Oxygen Therapy is used in severe COPD. Patients may require oxygen for day to day tasks, when sleeping or in some cases, 24 hours a day. Oxygen is available in tanks, liquid form or oxygen concentrators (COPD Support website, nd). Pulmonary Rehabilitation Programmes also plays a vital role in the treatment of COPD (Oireachtas website, 2013). In Ireland, programmes are normally ran over 20 sessions, with patients attending two to three times a week. Group numbers are limited to a maximum of 15 people. The programme consists of Exercise, Nutrition to increase fitness levels and Education to increase the patients awareness of the condition (Health Service Executive website, 2013). COPD patients have been shown to gain hugely from Pulmonary Rehabilitation Programmes, improving quality of life, reducing hospital admissions and increasing survival chances (Oireachtas website, 2013). A person with COPD uses 10 times the energy to breathe than a person without the condition. It is important to replace lost calories, maintain weight and also to protect the immune system against colds and flu’s (American Lung website, nd). Foods high in both energy, sugar and fat such as fried foods, full fat dairy products and readymade meals are recommended to encourage weight gain. However it is important that a healthy diet is also maintained (COPD Education website, nd). Supplement drinks are often recommended to increase calorie intake between meals. If swallowing issues occur, soft foods such as soups and stews are recommended and patients are encouraged to eat several smaller meals throughout the day (COPD Education website, nd). With this knowledge a HealthCare Worker would ensure that meals are small but nutritious at regular intervals . A HealthCare Support Worker would also consider how meals are prepared as softer, more manageable foods may be required. Conclusion This report has examined the causes, symptoms and treatments of COPD. This report discussed current statistics regarding the condition and some of the treatments available. From researching this report it is clear that more public awareness of COPD in Ireland is essential to highlight the link between COPD and smoking. COPD needs to feature strongly in Anti-smoking campaigns to promote this awareness, which would enable early diagnosis and treatment.

Behavioral Interviewing and Interrogation Techniques - Essay Example While he was standing in line, my cousin’s wife decided to go around the mall and just meet him somewhere after he was done. Around fifteen minutes after his wife left, my cousin left the bank. He called her on her cellular phone but there was no answer. So, he went to the usual shop he knew she liked. Five minutes later, his phone went off and it was his wife calling from the bank. He tells her to wait for him there. When they meet, he finds out that she had been on her way back to the bank, located on the first level, right after he had gone to the second level to look for her. They tried a newly-opened Italian restaurant for lunch. He got a medium-sized pizza for himself and she had an order of spaghetti. After eating, they headed to the supermarket to buy what they needed for the week. When they were done at the supermarket, they decided to head home so my cousin’s wife could take a nap. On the way home in the car, my cousin’s wife fell asleep and didn’t wake up until they got home. Since she was able to sleep a bit in the car, my cousin’s wife announced that she was making dinner early so she and my cousin could watch a movie at home before going to sleep. My cousin agreed. I think that the structural technique was better in this situation because I did not really want to know anything in particular. In the beginning, I used the traditional technique and asked the WH-questions. (Gosselin, 2007, p. 4-5) This is why I was able to know the date and time of some events. However, unless I asked something, my cousin would not give me additional information. Later on, I switched to the structural technique. This proved to be a lot better. I was still about to get information from my cousin. Moreover, he sensed that I was urging him to continue after his replies so he continued telling me his story. I mostly nodded to show him I understood what he was saying. I also asked follow-up questions which encouraged him to talk more. Since he was very
